Step 2: ACADEMIC TRANSCRIPTS Please fill out the Academic Transcript Request form and mail it, fax it, or scan and email it to the school(s) that you have attended. Feel free to make as many copies of the Academic Transcript Request form as you need. We must receive official copies of transcripts in sealed envelopes from each school. If you have a hold on your transcript with another institution, it will be your responsibility to contact that institution and remedy the hold with them. If you have any questions or concerns, please contact us. It is our pleasure to assist you as much as we can.
NOTE: If you received a GED, contact the Department of Education in the state where you took the test and ask that they have an official transcript of your scores sent to the LEAD Admissions Office. Please contact the LEAD Admissions Office if you need assistance with this.
NOTE: We require a minimum high school CGPA of 2.5 and a minimum college CGPA of 2.0.*
Step 3: TWO AUTOBIOGRAPHICAL ESSAYS & SIGNED ESSAY FORM
Answer one of the essay statements from each section
of the Autobiographical
Essays Form on a separate sheet of paper. The essays should be
typed and double-spaced. Or, if you prefer you can complete the essays online by CLICKING HERE. Please limit your responses to 500 words or
less for each essay. It is important that these autobiographical essays
are your own work and are factually correct. You must submit the signed
essay form with your Autobiographical Essays, verifying that it is your
Step 4: THREE REFERENCE FORMS
Fill out the top portion of the two Church
Leader reference forms and the Teacher/Employer
reference form. Hand out these forms and the provided Business Reply
envelopes to the three people who are referring you, and ask them to
send the forms to the Admissions Office. These references cannot
come from a family member, and need to have known you for at least one
For your convenience, we are able to complete reference forms over the phone without you having to worry about it. It is your responsibility to provide the name of the reference and his or her phone number. We will not call references unless you provide this information.
Step 5: ACT/SAT
Please have a copy of either your ACT or SAT scores sent to FCC. Our minimum ACT score requirement is 18 (Composite Score), and our minimum SAT score requirement is 870 (Math and Critical Reading score combined). If you have not already taken either of these tests, please arrange to take one of them as soon as possible and have the results sent to FCC. Our SAT Code is 2167 and our ACT Code is 0742. If your test scores are listed on your H.S. transcript, this is acceptable as well.*
NOTE: If you are transferring from another college with 12 hours or more of transferable credit, you will not be required to submit test scores and will be waived from this step.

application will be reviewed for acceptance. After they have been
reviewed, a letter will be sent notifying them of their acceptance
