Student FormsAdvisement and SchedulingAcademic Substitution Form- You must get approval to substitute another FCC course for a requirement. Procedure: Approval should be sought from your advisor, then the form should be turned into the Registrar's Office. It will then be forwarded to the appropriate Division Chair and approved or denied. Transient Student Form - You must get permission in order to take a course at another institution and replace any FCC course requirement or elective. Procedure: Approval should be sought from your advisor, then the form should be turned into the Registrar's Office. It will then be forwarded to the appropriate Division Chair and approved or denied. Grad Workup Request - A grad workup is your final degree audit provided by the Registrar's Office. It gives you the exact schedule you will need to complete your last year of study. Except in special circumstances, a grad workup will only be completed if approximately 40 credits or less are remaining in the student's degree program. Procedure: Fill out the request completely. Submit it to the Registrar's Office. Please submit requests at least 1 month prior to the next registration period. Request submitted after that time are not likely to be completed until after the registration period is over. Application for an Independent Study - Independent Studies are awarded only if extenuating circumstances exist. Procedure: Approval should be sought from your advisor, then the form should be turned into the Registrar's Office. It will then be forwarded to the appropriate Division Chair and approved or denied. Academic Appeals & PetitionsAcademic Petition (General) Petition for Incomplete - An incomplete grants an extension of 30 days past the course completion date. Incompletes are awarded only if extenuating circumstances exist. Petition for Exemption from Final Exams - This form must be filled out and submitted to the Registrar's Office by midterm of the semester for which the student is eligible to be exempt.
